    But except for one center of the first order - the sinus node, in the muscle tissue of the heart there are other areas that can send bioelectric impulses. These are automatic centers of the second and third order. And if the sinus node is broken, they take on the function of the drivers of the heart rhythm. The centers of the second and third order can not cope with the work as a sinus node does, they are much weaker, moreover, each of them sends impulses independently. As a result, the heart rhythm is disrupted, and there are such phenomena as paroxysmal tachycardia and other arrhythmias.

    In the complex work of the heart, his faithful assistant is the diaphragm, a tendon-muscular septum that separates the thoracic cavity from the abdominal cavity. The movements of the diaphragm regulate the flow of blood from the lower and upper hollow veins into the right atrium.

    The nature in all cases uses such means of increase of reliability, as duplication. If a person has two eyes or two ears that duplicate each other's work, why does he have one heart or one liver? And the harder we do it, the more help to the heart will come from the peripheral "hearts" - in fact, the more they train, the less the burden will fall on the main motor of our body.

    Training and hardening of the heart and its peripheral assistants must begin from childhood. Every day of every person should start with physical exercises. Those two lessons of physical education, which have children in schools, very, very little. This is the twentieth part of that physical load that the child's body must receive! It is especially difficult to recognize such a variant of myocardial infarction in patients with peptic ulcer and 12 duodenal ulcer in the anamnesis. However, palpation of the abdomen in this case does not cause significant soreness, the abdomen is soft, there are no symptoms of irritation of the peritoneum, which does not correspond to the patient's complaints for very severe pain.

    Myocardial infarction and acute surgical diseases can occur simultaneously.

    3. The cerebral variant is manifested as an acute disorder of cerebral circulation. There may be loss of consciousness, epileptiform cramps, speech disorders, paresis and paralysis.     Most often, the ECG changes described above occur in those leads whose positive( active) electrode is located above the necrosis zone.

    ECG changes:

    • in leads I and aVL indicates damage to the anterior wall of the left ventricle;

    • in leads III, aVF and( rarely) II - posterior diaphragmatic divisions:

    • in the thoracic leads: V1-V2 - anterior wall of the left ventricle, V2-V3 - interventricular septum, V4 - apex region, V5-V6 - lateral divisions of the leftventricle.

    The most difficult thing is to recognize the posterior basal myocardial infarction when registering an ECG in 12 leads, since in this zone the active electrodes are installed only in leads V7-V9.In the posterolateral basilar myocardial infarction, abnormally high R wave and depression( decrease) of the S-T segment in leads V1-V2 may occur.

    If the signs characteristic of myocardial infarction did not arise in the patient for the first time, but also on cardiograms made several months ago( and in the same way), one should think about the presence of chronic post-infarction changes( for example, scar formation or aneurysm).In doubtful cases, as well as with changes bordering the norm, the ECG should be repeated again after 6-8 hours( or later).

    In the presence of acute myocardial infarction, ECG changes will increase. In the first hours there may not be any changes at all, they will appear later, therefore, with characteristic clinical signs, it should be considered that the patient has myocardial infarction, and behave accordingly.
